Use of three gait-training strategies in an individual with multiple, chronic strokes
BACKGROUND AND PURPOSE: There is little information available regarding gait-training interventions for people with chronic, multiple strokes who are nonambulatory. The purpose of this case report is to describe the feasibility and medicare power wheelchairs, handicap scooters outcome of three different task-oriented gait-training techniques in an individual with chronic, multiple strokes who was not able to ambulate independently. CASE: The participant was a 61-year-old man with chronic quadriparesis resulting from five brainstem strokes sustained over five years previously. He maintained home and community mobility with a power wheelchair but sought to ambulate home and short community distances without assistance. Three six-week gait-training interventions were implemented sequentially twice per week: (1) overground gait training with manual assistance, (2) body weight-supported treadmill training (BWSTT), and (3) overground gait training with variable task practice. OUTCOME: Independent home and short community ambulation was not achieved by this individual after these interventions. However, he improved gait speed and endurance as well as balance over the 18-week intervention period. DISCUSSION: There appeared to be no advantage to BWSTT over traditional gait training for this individual, as improvement was noted during all three interventions at a comparable rate. Although these gait-training interventions facilitated gains in gait and balance measures during the treatment, no change in functional ambulation status was achieved. Given the time, financial, and labor demands involved with BWSTT and the lack of apparent benefit, traditional overground therapeutic intervention was reasonable and appropriate for this participant with a history of chronic, multiple strokes.

Evaluation of pushrim-activated power-assisted wheelchairs using ANSI/RESNA standards
OBJECTIVE: To determine and compare performance of pushrim-activated power-assisted wheelchairs (PAPAW) (iGLIDE, e-motion, Xtender) on national standards. DESIGN: Engineering performance and safety evaluation. SETTING: A Veteran Affairs and university-based research center. SPECIMENS: Nine PAPAWs. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Static, dynamic stability, brake effectiveness, maximum speed, acceleration, retardation, energy consumption, static, impact, and fatigue strength. RESULTS: There was no significant difference among the 3 models in forward stability. The iGLIDE was the most stable, whereas the e-motion was the least stable model in the rearward stability tests. All PAPAWs performed equally on the slopes of 3 degrees and 6 degrees in the forward and rearward directions. Braking distance was the highest for e-motion (5.64+/-0.28m) and the lowest (1.13+/-0.03m) for the iGLIDE in forward direction. The average equivalent cycles of all PAPAWs were 318,292+/-112,776.6 cycles (n=8) on the fatigue tests. All PAPAWs passed the impact and static strength tests. CONCLUSIONS: The standards of the American National Standards Institute and www.easymedonline.com, power wheelchairs the Rehabilitation Engineering and Assistive Technology Society of North America could act as quality assurance tool for wheelchairs. The standards for wheelchairs were first approved in 1990; after 17 years, exceeding the minimum values in the standards would be a reasonable expectation

Effects of training programs for spinal cord injury
INTRODUCTION: www.easymedonline.com exercise training programs in patients with spinal cord injury (SCI) were largely studied to determine different types of adaptations. The aim of specific rehabilitation is to obtain maximal gains in quality-of-life (QoL) after SCI. OBJECTIVE: To review the literature on the efficiency of training programs for SCI. METHODS: We searched the MEDline database with the keywords SCI, paraplegia and quadriplegia and synonyms, then combined them with one of the following terms: rehabilitation, training, exercise conditioning, physical fitness, exercise prescription, adaptation, effect, or benefit. We found 65 articles related to the physiological and psychological effects of training programmes on patients with SCI. RESULTS AND DISCUSSION: Training programs after SCI offer reconditioning cardiorespiratory, cardiovascular, cardiac, metabolic, bone, biomechanical, muscle adaptation, and QoL benefits. Reconditioning training increases VO2 max, reverses leg vascular resistance in the paralyzed legs and has possible cardiac and neural adaptations, favorable catecholamine responses and effects on platelet aggregation. Reconditioning can also modify lipid profile, reduce risk for cardiovascular diseases, prevent osteoporosis and increase maximal upper-extremity muscle strength, sprint power wheelchairs
output and maximal power output. This effect allows for considerable improvement in mechanical efficiency and wheelchair propulsion technique. CONCLUSIONS: Reconditioning training programs after SCI have a direct impact on function and QoL, permitting participation in physical activities in addition to daily living activities in subjects with SCI.

Reliability and validity of the power-mobility community driving assessment.
The www.easymedonline.com Community Driving Assessment (PCDA) is a performance-based measure designed to assess driving performance of individuals using power wheelchairs or scooters in community environments. This article reports the results of pilot testing and an evaluation of the assessment’s reliability and validity. Pilot testing was conducted with a random selection of Canadian occupational therapists working in the area of mobility. Although the response rate was very low, feedback confirmed the utility of the measure and contributed to one substantive scoring revision. Reliability and validity testing was conducted with a sample of 34 drivers. Internal consistency results were positive. Interrater reliability was fair to high but limited by the lack of variability in the scores. Construct validity hypotheses were tested on the relationships between PCDA scores and vision, perception, cognition, and environmental accessibility. Results indicated no relationships between the PCDA and perceptual and cognitive function and only a weak trend for a relationship with environmental accessibility. Concurrent validity was established: PCDA scores were positively associated with the judgments of therapists familiar with the driving performance of participants. In summary, the PCDA has moderate to good reliability, and content and concurrent validity results were found. More research is needed, particularly on the underlying constructs of successful driving performance. At this point, rehabilitation professionals and their clients are urged to use this assessment to establish driving performance rather than relying on assessments of perception, cognition, or environmental accessibility to predetermine whether someone will receive power mobility. Clinicians may find this a useful tool to identify where clients are able to drive safely in community settings, to identify specific learning needs, and, through those, to promote independent living for drivers of power-mobility devices

A Brief History of Peoria, Illinois
Peoria, whose name is derived from a Native American word meaning the land of fat beast, is located on the land originally inhabited by the Pe-orias, one of the five tribes that comprised the Illini confederacy. Assistive devices are now available that allow persons with severe physical disabilities to complete tasks independently. When the user has severe physical limitations, it may be advantageous to have an integrated control system where a single control interface (e.g., joystick, head switches, voice recognition system, keypad) is used to operate two or more assistive devices (e.g., power wheelchairs, augmentative communication devices, www.easymedonline.com, computers, environmental control units, and other devices that are controlled electronically). The advantages of integrated control are that persons with limited motor control can access several devices with one access site without assistance, and the user does not need to learn a different operating mechanism for each device. We reviewed research works pertaining to communication and environmental control, computer access, and wheelchair guidance systems. Power mobility driving training for seniors: a pilot study
This article describes two handicap scooters mobility training protocols used with seniors and compares posttraining driving performance. Twelve users of power mobility were consecutively recruited from two residential facilities in Toronto, Canada. The aim of training at both sites was to make clients comfortable with and safe at driving medical scooters devices. The content of training was similar, but training protocols differed significantly in terms of the number of sessions (means of 3.43 vs. 9.80; p < or = .05) and the time frame over which the sessions were offered (means of 1.57 vs. 5.10 weeks; p < or = .01). Participants at the two sites differed significantly in terms of overall driving performance (p < or = .05), gender (p < or = .01), and type of device used (p < or = .05). Overall, driving performance was significantly associated with facility, gender, type of device used, and training duration (p < or = .05). When these variables were entered into an exploratory hierarchical regression, facility accounted for 64% of the variance in driving performance. When facility was controlled for, the correlations between device and duration of training with driving performance were no longer significant. The determinants of driving performance are difficult to clearly specify as the variable facility encompasses gender as well as all other differences between the two training protocols. Nevertheless, these data provide direction for future research in this area.
